The Newfoundland and Labrador Medical Association and the Department of Health and Community Services have established a Family Practice Renewal Program (FPRP) to improve the effectiveness of family practice in the province, as part of a more integrated primary health care system to improve patient outcomes. As a core initiative of the FPRP, the RE-Boot Family Practice Network (FPN) encompasses a wide geography including the Burin and Bonavista Peninsulas and rural communities on the Avalon Peninsula. As such, the successful candidate should reside in a community on the Avalon Peninsula. The FPN is designed to improve patient care, to help family physicians work with their Regional Health Authority (RHA) to address common health care goals in their communities, and enhance professional satisfaction for physicians. The FPN is a non-profit corporation led by a physician Board of Directors.
Primary Responsibilities
The FPN Executive Director is a full time position reporting to the RE-Boot FPN Board of Directors, receiving supervision and direction through the Board Chair. The FPN Executive Director will use a community development approach to manage change, engaging physicians as FPN members, and supporting the membership in its collaboration with the RHA and other strategic partners, to address local patient care issues. This position will work with the Board to develop and implement plans, strategies and communication in support of FPN strategic goals. The Executive Director is expected to work independently, across FPN communities, and attend and support regular Board meetings.
Excellent leadership, facilitation, strategic thinking, and analytical skills are required, including ability to: develop programs, policies, and grant proposals; develop, manage and implement concurrent projects/budgets; engage and build effective relationships with physician members and other stakeholders; use good judgment to effectively handle sensitive or confidential matters and solve problems/conflicts; work in an unstructured environment; use flexible and creative approaches to solve problems; and juggle competing priorities.
Qualifications: University degree or equivalent in health/health administration/business administration, community development, relevant social sciences, or related field. Five or more years of progressive management experience, preferably in health care and/or the non-profit sector. Knowledge of the NL primary health care system including a good understanding of the RHA and primary care environment. Previous experience working with family physicians, other health professionals, or Boards of Directors would be considered an asset. Proficiency in the use of computers: Microsoft Word, Excel, Outlook, PowerPoint, and Internet navigation/research. Alternatively, a suitable combination of education and experience would be considered.

The Program Coordinator, Family Practice Networks (FPNs) is a full-time position, with an employment term of two years, reporting to the Senior Program Consultant responsible for FPNs. The Program Coordinator will work with the Sr. Program Consultant and FPRP team to help support development and operational needs of FPNs and FPRP as a whole.

The Program Coordinator, Practice Improvement, is a full-time position, with an employment term of two years, reporting to the Senior Program Consultant responsible for the Practice Improvement Program (PIP). The Program Coordinator will work with the Sr. Program Consultant and FPRP team to provide support for PIP initiatives and events offered by the central office, and FPRP as a whole.

Quality Improvement, Practice Management and Enhanced Access Initiative
Background
The Department of Health and Community Services (DHCS) and Newfoundland (NL) and Labrador Medical Association (NLMA) are committed to renewing the primary care system in Newfoundland and Labrador. This focus is a critical strategy in building a responsive, cost-effective, and sustainable health care system.
The Newfoundland Family Practice Renewal Program (FPRP) is a partnership initiative of DHCS and the NLMA leading the family practice renewal efforts in the province. The FPRP has developed a multi-prong approach to achieve this.
The Quality Improvement, Practice Management and Enhanced Access program is one of the key initiatives within the FPRP with a focus on developing a comprehensive and sustainable education and support program for family physicians and clinic staff. Health Innovation Group (HIG) and Memorial University of Newfoundland (MUN) have been contracted to deliver on this initiative.
The vision is to enable and empower family physicians and their teams to transform care for their patients through improved access, continuity, and health outcomes using the right quality improvement methods for their own practice. Practice Facilitators are key to this program, providing support to help practices in new and ongoing quality
improvement efforts.
Purpose & Scope of the PF Role
Practice Facilitators support primary care practices to achieve their goals for quality care. Equipped with practical tools and resources, this role will help physicians and staff to plan, test and implement improvements based on their unique context and the needs of patient populations. Practice facilitators will receive practical evidence-based training and will be part of a vibrant team focused on helping Newfoundland family physicians to address common challenges and improve processes. A team of practice facilitators will be contracted to work with physicians in all regions of the province. These regional roles will engage local clinics in improvement, and will be supported by leaders from Health Innovation Group and FPRP.
Qualifications and Requirements
Equivalent combination of education, experience and expertise may be considered.
● Undergraduate degree equivalent
● 5 years’ experience in a healthcare related sector (2 years’ experience working in primary care preferred)
● Experience with project management, change management, quality improvement and facilitation
● Experience with the Electronic Medical Record an asset
● Proficiency with Microsoft Office products
● Valid driver’s license with access to a reliable vehicle for travel

Duties & Responsibilities or Work Performed
1. Develop professional relationships with clinic teams, physician leaders/champions, and stakeholders
2. Engage physicians to learn more about this quality improvement initiative to help them achieve their goals for
better patient care, better work experience, and improved quality.
3. Facilitate the implementation of practice assessments to highlight gaps and opportunities. Identify and build on
strengths within each practice.
4. Understand clinic team goals for improvement and provide relevant support related to the initiative.
5. Introduce provincial quality improvement opportunities, programs and resources to clinic teams.
6. Guide teams through learning events, workshops and webinars.
7. Work with physician champions, motivate and inspire clinic teams to take action.
8. Leverage clinic team member diversity and structured approach to change to achieve clinic goals
9. Build quality improvement capacity in clinic teams using Model for Improvement
10. Support the implementation of quality improvement measures within clinics to sustain improvements.
11. Facilitate measurement and evaluation needs with clinic teams.
12. Support clinic teams to share their improvements and lessons learned with other teams to accelerate the spread
of improvements
Terms
● Reporting Relationship: Contracted role with HIG, with dual project reporting to HIG and FPRP.
● Type: Temporary Contract with a rate of $50-$55/hour depending on qualifications.
● FTE: Negotiable and flexible. We are seeking three candidates, part time and/or full time. Occasional evening
and weekend hours are required.

● Date Available: Negotiable (ideally candidate would be available for January 6, 2020)
● Term: Contract term to be between 1-2 years depending on initiative rollout

required, along with occasional travel to provincial locations.
